Kindly provide these documents for your booking at Genesis Clinic.

Your ID

Medical aid card & authorisation number (if on medical aid)

Your marriage certificate (if married)

A copy of your medical history from your midwife

Please note that you can only book at Genesis Clinic if you are under the care of one of the Independent Midwives affiliated with Genesis Clinic.
